What to Bring

In addition to personal items and informal clothing, we ask that you bring the following items.

General

*if you have a prescription, please bring a 30-day supply IN the prescription bottle *a photo id stating that you are from the jurisdiction that you live in (driver’s license, etc.) *a second form of identification (birth certificate, social security card, etc.) *health and prescription benefit card

Women with Children

• a schedule of all immunizations for each child
• a health and prescription benefit card
• a recent history and physical for each child by their family physician or pediatrician
• all medications for the child (in its prescription bottle)
• a letter from the pediatrician stating that the child is free from all communicable diseases
